The process

Getting started with support coordination usually follows a few simple steps. Here's what the journey typically looks like from first contact to ongoing support.

  1. 1

    Referral

It starts with a referral — from you, a family member, or a professional in your support network. You share some basic details about the participant and their plan so we understand how we can help.

  2. 2

    Intake Call

We arrange a friendly conversation to learn about the participant's goals, situation, and current supports. It's a chance to ask questions and check that support coordination is the right fit.

  3. 3

    Service Agreement

Once everyone's happy to proceed, we put a service agreement in place. It sets out what will be delivered, how coordination hours will be used, and what to expect from the working relationship.

  4. 4

    Ongoing Coordination

From there, we work alongside the participant — connecting supports, solving problems, and building capacity over time. We stay in regular contact and adjust as goals and circumstances change.
